I understand that attendance of a dog obedience training class is not without risk to myself, members of my family or guests who may attend or my dog because some of the dogs to which I/we will be exposed may be difficult to control and may be the cause of injury even when handled with the greatest amount of care or otherwise.

I hereby waive and release Susan Gillett, the landlord, employees (sub-contractors), owners and agents from any and all liability of any nature, for injury or damage resulting from the action or inaction of any dog or its owner or handler, or from the action or inaction of the landlord, Susan Gillett, or employees (sub-contractors), owners or agents, whether arising from negligence or any other reason or cause whatsoever and I expressly assume risk of any such damage or injury while attending any training session or function of Susan Gillett, or her employees or(sub-contractors), owners or agents, whether arising from negligence or any other reason or cause whatsoever.

I acknowledge that I fully understand the terms and provisions of the WAIVER, ASSUMPTION OF RISK AND AGREEMENT TO HOLD HARMLESS and that I am setting my hand delivering the same freely and voluntarily and unconditionally.
